How to Join the Provider Network |

Interested in joining one of our provider networks? Below we provide a summary of provider advantages for each network, and network provider applications. To begin the application process, download and mail a completed application form to Dental Network of America, Inc. (DNoA) and we will send to you the appropriate program provider agreement. The forms are included in the following sections:

Dental Preferred Provider Organization (DPPO) Network – A DPPO plan allows the covered individual greater flexibility in the selection of a dentist. Under the DPPO program, participating dentists agree to accept a discounted maximum allowable charge fee schedule as payment in full and agree not to balance bill the patient. Covered services are paid on a claims basis according to the dentist's contracted fee schedule, less any patient share costs, subject to annual maximums and deductibles. (Pre-treatment estimates are recommended for services exceeding $300.00). Advantages to joining the network include:

Dental Health Maintenance Organization (DHMO) Network – (Chicago, Illinois metropolitan area only) The DHMO plan offers participating dentists an excellent resource of patients. Whether you are looking for growth in your practice or you have available chair time, a DHMO plan may be just right for your practice. In a DHMO program, the members select the Primary Care Dentist from whom they wish to receive their care. The contracting dentists receive monthly payments based on member enrollment in their office. In addition, some benefit plans may require the member to pay co-payments for certain procedures to the provider. The contracting provider benefits from the following:
- Increased volume of patients
- Predictable and consistent monthly income from capitation payments
- Reduced administrative responsibilities – primary care dentists do not file claims for prepaid services
- Program orientation, on-going education and administrative support through DNoA's local Professional Services Representatives
- Toll-free Provider Support Line
- On-line 24-hour eligibility verification
- Streamlined referral process to specialty dentists

DNoA Select Referral Network – Even today there are many patients who are without access to dental insurance. To meet the needs of these patients who do not have coverage, Dental Network of America, Inc. (DNoA) offers the DNoA Select Referral Program. This program can be a valuable source of new patients and additional revenue to your practice. |

DNoA Select is a national dental referral program that offers members reduced fees of approximately 10% to 40% on various dental procedures. Members, not DNoA, will pay you directly at the Select Referral rates. This is a discount program and NOT an insurance program; therefore, there are no claims forms, deductibles, or waiting periods. This program is especially attractive to associations or credit card companies looking to offer its members additional benefits.
Participants have access to over 15,000 network dentists nationwide. Some of the advantages of participating in the DNoA Select program include:
- Increased volume of patients
- Marketing of practive through DNoA directories and the website
- Ease of administration since DNoA Select is NOT an insurance program. (There are no claim forms, deductibles, waiting periods, pre-certifications or annual maximums, which means less paperwork for your staff. An identification card bearing the DNoA Select logo is presented at the time of service, allowing immediate savings for the member, who will pay you directly)
