Our voluntary dental insurance solutions are offered by Fort Dearborn Life Insurance Company (FDL), a sister company of Dental Network of America, Inc. (DNoA). FDL offers two types of benefit plans to employers: |

- Scheduled Benefit Plan – available to groups with minimum participation of two (2) enrolled employees
- Reasonable & Customary Benefit Plan (R&C) - available to groups with minimum participation of 30 enrolled employees

DNoA provides full service dental administration for these voluntary insurance plans which includes prompt accurate claim payments, coupled with integrated single source billing and eligibility. |
